We are located inside Ansley Mall, which is at the intersection of Piedmont Ave and Monroe Dr in the Ansley/Midtown area of Atlanta. Park directly in front of Pet Supermarket and walk down the breezeway past Cooks Warehouse to Suite 402 on the right.
If you are experiencing a medical emergency please call 911. If you would like to make an appointment, please contact our office by phone or email. Please call our office if you need immediate service. If you would like to request an appointment online, please select the ‘Appointment Request’ option from the menu above. A member of our staff will follow up with you directly. Appointments are requested during all office hours. Walk-ins are only accepted on an emergency basis.
Our practice accepts cash, checks, Visa, MasterCard, American Express and Discover. Co-payments and deductibles are expected at the times of service. Insurances accepted include United Healthcare, UMR, First Health, Blue Cross Blue Shield, Cigna, Humana, Aetna, Medicare, Multiplan and more. We are not in-network for Ambetter or Medicaid. Please call your insurance company to verify if we are a participating provider for your insurance network. You are also responsible for obtaining any necessary medical referral for care in our office BEFORE your first visit.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
